As Health Coach Manager, you are responsible for managing a team of coaches that are providing direct care for our patients. Health coaches at Virta work directly with patients to help reverse metabolic disease by providing high quality remote care through personalized nutrition and lifestyle recommendations. You’ll lead, develop, and inspire your team to achieve exceptional patient outcomes and ensure delightful patient experiences as our teams continue to grow and scale.

Responsibilities
<ul>
<li>Lead a diverse team of health coaches and own team metrics and outcomes, while ensuring safe and high quality delivery of the Virta Treatment, managing coach capacity and team coaches, and supporting individual coach development</li>
<li>Create and maintain a collaborative environment of trust, problem solving, and growth.</li>
<li>Encourage opportunities for teammates to collaborate, mentor each other and share learnings and best practices to elevate patient care and achieve patient success.</li>
<li>Monitor and own coach (and team) performance through key metrics to Identify high performers and actively manage underperformers through coaching and performance management. Providing guidance, coaching, and consistent feedback to all teammates.</li>
<li>Assist and support coaches as an expert in our coaching methods, internal systems, processes, and technology</li>
<li>Encourage a solution-oriented culture and work to iterate and improve on our processes</li>
<li>Lead teammates through transitions and times of change with clear communication, empathy, and understanding while helping teammates understand what success looks like.</li>

90 Day Plan
Within your first 90 days at Virta, we expect you will do the following:

<li>Master the basics of the Virta Treatment and understand the full patient experience.</li>
<li>Become an expert on our coaching methods, internal systems, processes, and technology.</li>
<li>Get to know your fellow coach managers and direct reports.</li>
<li>Partner with other Coach Managers to build relationships, share learnings and provide consistency for all of our patients.</li>
<li>Understand key performance indicators that are used to track health coach and pod (team) performance and what behaviors influence those metrics.</li>
<li>Complete audits and chart reviews to provide developmental feedback to teammates.</li>
<li>Review patient feedback and follow up with patients to identify opportunities to improve the patient experience through coaching.</li>

Must-Haves
<ul>
<li>3+ years people management experience&nbsp;(required)</li>
<li>Bilingual, (English/Spanish preferred)</li>
<li>Experience in a digital health organization or customer service support role (strongly preferred)</li>
<li>Prior experience (1+ year) coaching or counseling patients in a clinical and/or behavioral setting (strongly preferred)</li>

Registered Nurse Care Coach – Part Time

Overview:
This is a remote role.&nbsp;CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program. In this&nbsp;part time role (20-25 hrs per week),&nbsp;an RN Care Coach will be assigned a group of patients that they will be following and calling each month. In these monthly calls you will provide education, coordinate care, close preventive care gaps, and coach on strategies for self-management to keep them out of the hospital.
Key Responsibilities:
<ul>
<li>Utilize our specialized care management software to call Medicare patients with 2 or more chronic conditions (Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis</li>
<li>Build and maintain rapport with patients to help coach them to improved health through SMART goals and education on self-management strategies</li>
<li>Implement and improve the Plan of Care by updating medications, appointments due, biometrics, symptoms, and interventions made</li>
<li>Connect the patient with community resources as needed, including transportation, personal care needs, prescription/DME assistance, social services, etc.</li>
<li>Conduct Transitional Care Management activities to high risk patients discharged from the hospital and the ER to reduce unnecessary readmissions.</li>
<li>Close care gaps by encouraging and assisting with preventive care measures, i.e. annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.</li>
</ul>
Required Skills and Abilities:

Compensation:
Care Coaches can earn up to $45.00 per hour. Compensation is paid at the rate of $15.00 per initial Clinical Encounter per patient per month. A clinical encounter occurs after two criteria are met: a patient has a successful clinical call and the patient has 20 minutes or more of time in their chart timer.
<ul>
<li>Ex: If in one hour you called and spoke 3 patients and spent 20 minutes with each of them, your pay for that hour would be $45.00 per hour ($15.00/pt reached x 3)</li>
<li>Ex: If in one hour you called and spoke with 2 patients and spent 30 minutes with each of them, your pay for that hour would be $30.00 ($15.00/pt reached x 2).</li>
</ul>
Pay Timing:
Monthly via direct deposit, 40 days after the last day of the month of service. This is due to the time it takes Medicare to process reimbursements, but your monthly pay is guaranteed after the month is over.

The Role

<li>The Bloomerang Database Coach will report to the Database Coaching Manager and will be primarily responsible for the delivery of Bloomerang Coaching Sessions and Success Programs with a focus on best fundraising and database practices. Coaches strive to help customers overcome current database obstacles and to have a customer leave a session feeling more comfortable and confident in their ability to work independently in their database in the future. This person will play a crucial role in customer success, retention, and adoption assisting customers with their day-to-day fundraising database efforts.</li>

<li>Goal of at least 2-3 coaching sessions per day during the week.</li>
<li>Develop clear agendas based on the customer’s requests and goals for the session and send appropriate scheduling, pre-meeting and post meeting communication with customers.</li>

<li>Actively reach out to Success Program participants and check on their progress within the program.</li>

<li>Assist Database Coaching Manager with keeping track of customer communications and trainings scheduled within the CRM.</li>
<li>Create content and manage the Customer Facing Learning Management System.</li>
